It’s not in all watch os 4 watches
It’s not on watchOS 4 at all. You have to have a Series 4 Apple Watch running watchOS 5.1.2. Also currently the USA is the only country allowing its citizens access to this app.
Just hope ECG app comes to European Union asap. It was supposed to launch at the same time as the US version.

So if you are on the beta of 5.1.2 how do you get it? Because ECG wasn’t in the beta and the watch still thinks it’s up to date.
Remove your beta profile. Restart the Phone. Check for updates.
Does this work on iPhone 7? I updated my watch 4 but when I open the health app …….. crickets Nothing opens asking me to set it up. Running iOS 12.1.1
It shouldn’t matter what iPhone you have. Are you in the USA? So far this is the only government to allow it. There should be an ECG app on your watch

You should also mention that ECG is currently only available in the US and to Apple Watch Series 4 devices bought in the US. You cannot location spoof in any way currently and even if you live in the US you need to have a US bought device.

Not true man. I live in Costa Rica and bought an Apple Watch series 4 here in Costa Rica and the ECG function works just fine. Nevertheless I should mention that I had to change the location in my iPhone and Apple Watch to make it works.
Problema. I have iPhone X, but the apple health app did not open with the set up for ECC. Why? When I open the app Health, the same old screen….

so I am an american and connected to the US app store, but I am in Thailand and bought my apple watch series 4. I Do not see the ECG app on my watch?
